ESA WCA DescriptorsThere are large areas to write in the detail, and you have really roughly explained how you are in the above post.
In these areas give examples of how you are effected .You have stated that it varies , so fill in details and examples in the larger boxes.Try and use as much of the white space as possible .
Something that the form does not mention , but the legislation does , is repeatability , so if you can mobilise 50 metres but cannot repeat this with a reasonable amount of time , then say so .
If you have trouble writing and need a form that you can type into , then you can use the form on Direct.gov , then save and print and sign . You will then also have a copy on your computer for future reference.

HereGet GP/ Consultant evidence now , as this could sway any decision without having to appeal.
Remember you can appeal a decision within one month of that decision.
